We're happy to accommodate walk-in patients during our regular business hours-no appointment necessary.

To minimize wait times, we recommend submitting your lab orders in advance via email or fax.

How to Schedule a Lab Appointment

1. Visit Our Website: Navigate to the Schedule Lab Draw section.

2. Provide Patient Information: 

   • Full name

   • Email Address

   • In the notes section, indicate if the paper order will be brought in person, emailed, or faxed by your provider

3. Confirm Appointment: Review all details and submit. You'll receive confirmation via email

How to Submit Paper Orders by Email

1. Obtain the Order: 

   • Ensure it's signed by your provider

   • Include clearly listed diagnosis codes (ICD-10)

2. Scan or Take a Clear Photo: Use a scanner or smartphone to create a legible image or PDF.

3. Email the Order: Send to: info@openskylaboratory.com

4. Get Confirmation: You will receive an email once your order has been reviewed.

Providers Can Submit Orders

You may request that your provider fax lab orders directly to us at:

Lab Appointment Requirements

• A signed paper order from your provider, including diagnosis codes (ICD-10)

• Insurance card (if available - not required)

• Accurate contact information for follow-up or scheduling changes

Lab Results

• Lab results will be sent directly to the ordering provider on your orders
